Subject: Union County Registration Information for School Year 2006-2007
The Tennessee Department of Education requires School Systems to keep records of immunization, physical examination, and date of birth for each student who is enrolled in school.
To enter school for the first time during the 2006-2007 school year, proof of the following is required:
1. Birth date 2. Physical Examination 3. Immunization 4. Social Security Card 5. Other Information
This proof is to remain in permanent files in the school. You may pick up required forms at the Health Department if they are not available at your doctor's office.
Children who are five on or before September 30, 2005 are to enter kindergarten during the 2006-2007 school year.
Children who are six on or before September 30, 2005, and who completed kindergarten are required to enter first grade for the 2006-2007 school year.
The following immunizations are required for school entry. Any record having fewer than the minimal requirements shall be counted as deficient.
| DTP-DTaP-DT-T-Td | 4 or more doses, one of which must be given on or after the fourth birthday. If starting series at age seven or older, only three doses are required. |
| Polio (OPV/IPV) | 4 doses of OPV or IPV. If the 3rd dose was given on or before the fourth birthday, the 4th dose is not required. However if a combination of IPV/OPV (2IPV/20PV) is used, all 4 doses are needed regardless of age. |
| MMR | 2 doses given on or after the first birthday, for entrance to all grades effective July, 2001. The second dose must be administered at least 30 days after the first dose. |
| Hepatitis B | 3 doses, for entrance into Kindergarten |
Chickenpox vaccine is required for entrance into kindergarten and
Hepatitis B vaccine is required for students in grade 7.
You will need the following information to register:
1. Copy of immunization signed by medical provider using Permanent Tennessee Certificate of Immunization (white form).
2. 2. Copy of recent physical examination signed by medical provider using Tennessee School Entry Medical Examination Form
(white form).

Kindergarten registration for school year 2006-2007 will be held at all Union County Public Elementary
Schools on Thursday, May 4, 2006, 3:30 to 6:00 P.M. Please bring all required information to register
your child at the Union County Public School he/she will attend in 2006-2007.
